Since mid-2018 Discovery Health no longer accepts the change of banking details form to change banking details. This needs to be done via the Discovery website. Please contact us if you require assistance with this.

The following supporting documentation is required with the claim form:

–       Hospital Account

–       All relevant doctors Accounts

–       Medical Aid Claims Transaction History for the procedure or hospital stay.
